During the evaluation, a licensed therapist will evaluate and identify your needs. Areas being evaluated may include any or all of the following: speech, language, voice, fluency, swallowing, feeding, orthopedic, neurologic, or sensory disorders.   After the evaluation, the therapist will review findings and whether or not therapy is warranted.  The evaluation report will be sent to the primary care provider for review and approval.  From that point on, therapy (if warranted) will be set up on a consistent basis.

 Before your initial appointment, please click on the paperwork link to download, print, and fill out before your appointment.  Through this link you  will find our general case history form which educates us on your current status.


If you are a current or past patient of The Speech Path and need to request a copy of your therapy records, you can do so by downloading the following form.  Make sure to fill it out completely and fax it back to us at the number on the bottom of the page.  If you have any questions regarding this form, please let us know.  All records requests will be handled within 2 business days. 

 Your road to success begins with an initial evaluation. To schedule an evaluation, we first need a referral from your primary care provider.   You can ask them to fill out the referral form below.  We will contact you after we receive this form to schedule an appointment.
