Your road to success begins with an initial evaluation. To schedule an evaluation, we first need a referral from your primary care provider.   You can ask them to fill out the referral form below.  We will contact you after we receive this form to schedule an appointment.

Please show up 15 minutes early with your paperwork already filled out.  The Patient Handbook provides helpful information about our policies and procedures. 

Patient Handbook

During the evaluation, a licensed therapist will evaluate and identify your needs. Areas being evaluated may include any or all of the following: speech, language, voice, fluency, swallowing, feeding, orthopedic, neurologic, or sensory disorders.   

After the evaluation, the therapist will review findings and whether or not therapy is warranted.  The evaluation report will be sent to the primary care provider for review and approval.  From that point on, therapy (if warranted) will be set up on a consistent basis.
